Making an appointment

To make an appointment for a consultation with a cardiologist or to book a test please call our friendly staff on (02) 9460 8700.

You will need a valid referral from a General Practitioner or Specialist. To be eligible to claim a Medicare rebate you need to be both eligible for Medicare and have a current referral.

A referral from a General Practitioner will be valid for a period of up to 12 months. After 12 months it is essential for you to provide a new referral to meet Medicare requirements.

If you have been referred by a Specialist your referral will be valid for three months. Please contact our staff if you are unsure of your referral status.

Attending an appointment

  1. Make sure you have a valid referral this will ensure you can claim Medicare rebates and inform the cardiologist of the background of your condition.
  2. If you are attending for the first time, please arrive 10 minutes early to fill out the patient information and privacy consent form. Alternatively, please print and complete the attached form and bring it with you, to save you time on the day.
  3. For billing and Medicare rebate purposes it is essential that you bring your Medicare/DVA and concession cards to the appointment.
  4. It is very important that your cardiologist knows what medications you currently use. The safest way to be sure we have the correct information is to bring the medications to the appointment or take a photo of them.
  5. Scans, pathology and reports which relate to your condition should also be brought to appointment.

If English is your second language and you require an interpreter please let our staff know when you make your appointment.

A reminder notification will be sent via SMS 3 days before your appointment. Please respond to this SMS to confirm your attendance with a Yes or No. If you are unable to attend the appointment, please call our office if you would like to reschedule.
